What is Nursing Home Care?
Nursing Home Care is for clients who are chronically ill or disabled. These clients have a chronic illness or disability and need long-term care.
Nursing home care is divided into two sections:

Psychogeriatric care: for clients who need care due to an illness of the brain, like, for example, Alzheimer’s disease.

Somatic care: for clients who need care due to a physical illness or disability, like, for example, after stroke or amputation. To be eligible for admittance, clients need to have an ADL (Activities of Daily Living) score of 5 or lower.
Admission criteria
Required documents:

Copy of valid health insurance

Copy of SXM ID card /passport

A score of 5 or lower for the Activities of Daily Living (assessed by us)

A detailed census registration form (no older than 6 months)

Letter from a psychiatrist confirming
a psycho-geriatric illness (if applicable)

Medical history from the family doctor / medical specialist

Follow up
Once all documents are submitted, the client will be placed on the waiting list.
The social worker and department manager will then conduct a home visit, where the client’s needs are assessed. An AVBZ-care request is then filled in and submitted to the AVBZ Committee at SZV for approval.
Once approved and when a space becomes available for the client, the family will be called and an admission date is planned.
