What is Nursing Care?
Nursing Care provides short-term or long-term care to clients who are getting older, chronically ill, recovering from surgery, or are disabled. We provide care to clients of all ages to remain safely at home. Nursing Care offers help with basic care like bathing, getting dressed, and eating and skilled care given by practical and registered nurses. For example, catheter care, respiratory treatments, tube feeding care, intravenous medication and injection administration, and guidance for terminally ill.

Why Nursing Care?
Nursing Care allows clients to stay safely in
their homes. It helps avoid unnecessary
hospitalization by offering a more client-centered
solution. Interacting with loved ones in their own
environment keeps clients active and mentally
stimulated.

Receiving specialized Nursing Care is the key to achieving the highest level of recovery and quality of life. Knowledgeable and experienced nurses will care for clients with various conditions. Compared to the hospitalized client, the Nursing Care client often has a greater role in determining how and even if specific interventions will be implemented.

Admission criteria
The following documents are needed to start the intake process:

Referral letter with a clear description of the care that is required submitted via Hecina (SZV portal)

Medication list (if applicable);

Wound treatment plan (if applicable);

Copy of valid ID card;

Copy of valid insurance card;

Contact information of the client, family member, or caretaker.
In case a client transfers directly from a hospital and was admitted for more than 24 hours, additional documents must be emailed to our team via: districtnursing@wyccf.org

Updated treatment & transfer report

Recent HRMO throat, rectum (and if applicable sputum, urine, skin lesions, and wounds) results

​Recent MRSA nose, throat, perineum (and if applicable sputum, urine, skin lesions, and wounds) results

Follow up
Once the referral is received via Hecina (SZV), we will contact the potential client or family for an intake at home. The total care needs are assessed, and a care contract is made and sent to SZV for approval.
After approval, the care can start as soon as the supplier contracted by SZV delivers all materials, and space is available.
