What is Home Care?

Home care means assistance with activities of daily living such as bathing, dressing, and grooming, but also assistance with household tasks. It entails short-term or long-term care for clients who are getting older, chronically ill, recovering from surgery, or are disabled.

Our nurses help clients of all ages who need additional support to remain safely at home. They can assist with meal planning, grocery shopping, and, if necessary, support adequate medication use by administering medications, picking up prescriptions, and monitoring vital signs. The Home Care nurse also supports meaningful companionship and peace of mind for clients and their families by engaging clients in hobbies and activities or escorting them to appointments.

Why Home Care?

Home care helps to avoid unnecessary hospitalization, offering a more client-centered solution. Clients are in their own environment and interact with people they love. It keeps clients active and mentally stimulated. The increased independence maintains a high quality of life. Home Care can also delay or prevent admission into a nursing home or elderly care facility.

Admission criteria

Home Care is only covered via an Island Government doctor card under SZV (OZR fund)

 

The following documents are needed to start the intake process:

  • Referral letter with a clear description of the care that is required submitted via Hecina
    (SZV portal) for OZR insured only.

  • Medication list (if applicable);

  • Wound treatment plan (if applicable);

  • Copy of valid ID card;

  • Copy of valid insurance card;

  • Contact information of the client, family member, or caretaker.
     

 

 

In case a client transfers directly from a hospital and was admitted for more than 24 hours, additional documents must be emailed to our team via: districtnursing@wyccf.org

  • Updated treatment & transfer report

  • Recent HRMO throat, rectum (and if applicable sputum, urine, skin lesions, and wounds) results

  • ​Recent MRSA nose, throat, perineum (and if applicable sputum, urine, skin lesions, and wounds) results

Follow up

Once the referral is received via Hecina (SZV), we will contact the potential client or family for an intake at home. The total care needs are assessed, and a care contract is made and sent to SZV for approval.

After approval, the care can start as soon as the supplier contracted by SZV delivers all materials, and space is available.
